Elderly case management is a process of integrating medical management. By assessing, planning, implementing, coordinating, monitoring and evaluating the needs and resources of various medical and health services for the elderly, considering the cost, effect and quality of the entire medical, nursing, rehabilitation and social support service system, and communicating resources of all parties, the elderly can not only receive timely and effective personalized services, but also receive timely and effective personalized services. Better medical results and quality of life can be achieved, and more importantly, social resources can be saved and integrated to reduce the overall expenditure on health services for the elderly. The geriatric integrated care manager, also known as the case manager, plays an important role in implementing this service. Since 2019, the Chinese Society of Geriatrics has taken the lead in applying for and carrying out this national continuing education project, hoping to make further progress in the construction of interdisciplinary geriatric team talents and the implementation of policies such as the integration of medical care and long-term care in addition to the existing national continuing education programs such as advanced geriatric clinical skills training and geriatric nurse training for geriatrics physicians. The director of the project, Hu Yixin, director of the International Exchange and Cooperation Department of the Chinese Geriatric Society, introduced that the geriatric integrated medical manager (also known as the case manager) is one of the important members of the interdisciplinary geriatric team. Overseas community service agencies, hospitals or nursing homes, and insurance companies all play an important role in assisting doctors to formulate individualized health management plans, educate the care recipients and their families about relevant care knowledge and skills, integrate and utilize social resources, and effectively provide holistic care services for the elderly. However, in different work places, the requirements for their skills training are different; There are a variety of academic societies or social organizations in the United StatesImplement the skills training and qualification certification of elderly case managers, and have a mature training system. At present, with the need for long-term care of the elderly in our country, all walks of life have gradually recognized the role of geriatric assessors. However, to truly realize the comprehensive assessment of the elderly, coordinate resources and assist doctors in developing treatment and care plans for the elderly, talents with integrated medical management skills, that is, case managers, are needed to complete. The training of talents in this field is still blank in our country. To this end, this project has been held for three periods since 2019. The design course system not only refers to foreign theoretical systems, but also continuously improves and adjusts according to the characteristics of China's medical, insurance and care systems, which is welcomed by many medical and nursing combination structures in society and geriatric departments in hospitals. This project, with the support of the leaders of the Association, the leaders of Zhengzhou No. 9 Hospital and experts from all walks of life, focuses on exploring the training system of geriatric case managers in the hospital, working with all walks of life and advancing with The Times, and making contributions to the multidisciplinary team construction of geriatric integrated medical service talents.
